Bridgeport Hospital: Benefits Plans

Medical Plan Options
Administered by Anthem Blue Cross & Blue Shield.
No referrals required.

  • Yale New Haven Health Advantage Plus Plan
    Providing in- and out-of-network coverage.
    • Highest level of benefits when services are provided at a Yale New Haven Health System hospital.
    • In-Network Benefits: $20 copay office visits, and 100% coverage for most services. No claim forms required.
    • Out-Of-Network Benefits: 70% coverage for many services following $350 individual/$700 family deductible.
  • Yale New Haven Health Advantage Plan
    Offering lower per pay contributions, but you must meet a deductible if you need major health care services.
    • In-Network Benefits: Preventive care covered at 100% following $20 copay with no deductible. Physician/specialist office visits are $30 copay with no deductible. All other services are covered at 80% after $200 individual/$400 family deductible.
    • Out-Of-Network Benefits: $750 individual/$2,250 family deductible; 60% coverage for most services.

Prescription Drug Program

This plan is included with your medical plan election and is administered by CVS/Caremark. On a per-prescription basis, employees pay 10% ($8 minimum, $23 maximum) for generic drugs, 20% ($20 minimum, $45 maximum) for preferred brand drugs, and 40% ($30 minimum, $60 maximum) for other brand drugs at participating pharmacies. There is no coverage at out-of-network pharmacies.

Up to a 30-day supply of drugs may be provided. Employees taking maintenance drugs must use mail order — which is available at $18/$45/$68 copays for a 90-day supply.

Dental Health Options

  • Delta Dental Basic Plan
    Covers 100% of preventative services with no deductible, and 80% of restorative services after a $50 individual/$100 family deductible.
  • Delta Dental Plus Plan
    Covers 100% of preventative services with no deductible, 80% of restorative services, 50% of major services, and 50% of orthodontic services after a $50 individual/$100 family deductible with a $1,700 lifetime maximum benefit.

Opt-Outs

Employees who have medical coverage through another group plan and are electing to waive our medical coverage are eligible to receive annual Flex credits of $525 (for Full-Time employees) and $425 (for Part-Time employees). If your spouse is covered by any YNHHS medical plan, or if you are covered under Title 19, you may not receive the opt-out payment.

Vision Service Plan (VSP)

An optional program for benefits-eligible employees, providing exams, contacts, frames and/or lenses at affordable prices. VSP network doctors provide top quality care and offer a wide selection of frames to choose from, all in one convenient location. Flexible Spending Accounts

Benefits-eligible employees may elect to have a portion of their salary deducted on a pre-tax basis to pay for medical and/or dependent care expenses. Use of these salary conversion dollars also reduces the amount of gross income subject to income taxes and social security.

Life Insurance

Bridgeport Hospital automatically enrolls benefits-eligible employees in life insurance equal to one times their annual salary (to a maximum sum of $50,000) at no cost. Employees also have the option of purchasing (with post-tax dollars) additional insurance up to three times their annual base salary, to a maximum of $500,000. Spouse & child life insurance is also available.

New Hire Benefits

Bridgeport Hospital has contracted to make medical coverage available to new hires during the enrollment waiting period for this plan. Contact the Benefits Department for more information.
